A term of long-standing use and variable meaning, generally taken to refer to chronic continual drinking or periodic consumption of alcohol which is characterized by impaired control over drinking, frequent episodes of intoxication, and preoccupation with alcohol and the use of alcohol despite adverse consequences.

The term alcoholism was originally coined in 1849 by Magnus Huss. Untill the 1940s it referred primarily to the physical consequences of long-term heavy drinking (beta alcoholism in Jellinek's typology). A narrower concept is of alcoholism as a disease (see alcoholism, disease concept of) marked by loss of control over drinking, caused by a pre-existing biological abnormality, and having a predictable progressive course. Later, the term was used by Jellinek and others to denote the consumption of alcohol leading to any type of harm (physical, psychological, or social; individual or societal). Jellinek subdivided alcoholism thus defined into a series of “species” designated by Greek letters (see Jellinek's typology).

The inexactness of the term led a 1979 WHO Expert Committee* to disfavour it, preferring the narrower formulation of alcohol dependence syndrome as one among a wide range of alcohol-related problems. Alcoholism is not included as a diagnostic entity in ICD-IO (see dependence syndrome).

Despite its ambiguous meaning, alcoholism is still widely employed as a diagnostic and descriptive term. For instance, in 1990 the American Society of Addiction Medicine defined alcoholism as “a primary, chronic disease with genetic, psychosocial, and environmental factors influencing its development and manifestations. The disease is often progressive and fatal. It is characterized by continuous or periodic: impaired control over drinking, preoccupation with the drug alcohol, use of alcohol despite adverse consequences, and distortions in thinking, most notably denial.” Other formulations have split alcoholism into various types, some regarded as diseases and some not (see Jellinek's typology). Distinctions are made between essential alcoholism and reactive alcoholism, where “essential” indicates that alcoholism is not secondary to or precipitated by some other condition; between primary and secondary alcoholism, to indicate the order of onset in cases of dual diagnosis; and between Type I and Type II alcoholism, the latter having a male-linked, strongly genetic component. In older usage, dipsomania (episodic drinking) and alcohol addiction referred to loss-of-control drinking: inebriety also had a broader reference to habitual intoxication and its harmful effects.


 長年にわたって様々な意味に使われている用語であるが、一般的には、飲酒のコントロールの減弱、頻繁に繰り返される酩酊のエピソード、有害な影響があるにも関わらずアルコールおよび飲酒に没頭する、という特徴を持った、慢性的で継続的あるいは周期的なアルコール消費を指す。

 アルコホリズムという言葉は、もとは1849年にマグヌス・フス(Magnus Huss)によって作られた言葉である。1940年代までは、主として長期にわたる大量飲酒が身体に与える影響を指した(ジェリネックの類型学でβ型アルコール症(beta alcoholism)とされたもの)。

 疾病としてのより狭い概念のアルコホリズムは、飲酒のコントロールの喪失によって特徴づけられるもので、生来的な生物学的異常を原因とし、予測可能な進行の過程を取る(アルコール症の疾病概念を参照)。

 後に、ジェリネックその他によって、どのような種類であれ(身体的、心理学的、もしくは社会的;個人的もしくは社会的)害をもたらすアルコール消費を指して使われるようになった。ジェリネックはアルコール症を、一連のギリシャ文字を付けた型(species)に細分化して定義した(ジェリネックの類型学を参照)。

 このように厳密さを欠いた用語であったため、1979年のWHOの専門家委員会ではそれを嫌い、広範囲に及ぶ「アルコール関連問題」の中のひとつとして、狭く明確にされた「アルコール依存症候群」を好んで使用した。1 ICD-10にはアルコホリズム(アルコール症)は診断単位として含められていない(依存症候群を参照)。

 アルコホリズムは、その意味の曖昧さにもかかわらず、診断及び説明の用語として依然として幅広く使われている。例を挙げると、1990年に American Society of Addiction Medicine(アメリカ嗜癖医療学会)は、アルコホリズムを「一次的、慢性の疾病で、その発病と症状には遺伝的、社会心理的、環境的要因が影響する。この疾病はしばしば進行性で致死性である。継続的あるいは周期的な、飲酒のコントロールの減弱、アルコールという薬物への没頭、有害な影響があるにも関わらずアルコールを使用する、思考の歪み、とりわけ目立つものとして否認、によって特徴づけられる」と定義した。

 アルコホリズムを複数のタイプに分けて記述したものもある。あるタイプは疾病として、他のタイプは異なるものとみなしている(ジェリネックの類型学を参照)。本質性アルコール症(essential alcoholism)と反応性アルコール症(reactive alcoholism)の区別は、本質的アルコール症は二次的なものではなく、何らかの他の状態によって引き起こされたものではないことによる。一次性アルコール症(primary alcoholism)と二次性アルコール症(secondary alcoholism)は、多重診断が下された際にどちらが先に始まったかの違いを示す。タイプ1とタイプ2のアルコール症の違いは、後者が男性の遺伝的成分に強く関連していることである。

 古くは、コントロールを喪失した飲酒に対して、dipsomania (episodic drinking) =渇酒症(挿間的飲酒)あるいはアルコール嗜癖(alcohol addiction)が使われた。また習慣的な酩酊やその有害な影響を指して 飲酒癖(inebriety)という言葉も用いられた。

Until recently, there has been a widespread tendency to conceptualize the whole gamut of alcohol problems as manifestations of an underlying entity, alcoholism. Undoubtedly a wide variety of problems are related to the development of the “alcohol dependence syndrome” (see Annex 2). It should be pointed out, however, that there are many physical, mental and social problems that are not necessarily related to dependence. Alcohol dependence, while prevalent and itself a matter for serious concern, constitutes only a small part of the total of alcohol-related problems.

The term “alcohol dependence syndrome”, which is described in some detail in the report of a WHO Group of Investigators,1 has replaced the rubric “alcoholism” in the ninth revision of the International Classification of Diseases 2 and is increasingly coming into use for scientific purposes. According to the aforementioned report, 1 one criterion for diagnosis of the alcohol dependence syndrome is a changed behavioural state in an individual that includes, in addition to an alteration in overt drinking behaviour, a continuation of drinking in a way not approved of in his culture, despite painful direct consequences, such as physical illness, rejection by his family, economic embarrassment and penal sanctions. The report refers also to an altered subjective state, in which the dependent person's control over his drinking is impaired, there is a craving for drink, and an element of “drink centredness” is manifested, whereby the planning of his drinking may take precedence over that of other activities. In addition to the above changes, an altered psychobiological state is noted, with experience of signs and symptoms of withdrawal, drinking for relief of withdrawal, and increased tolerance.

4.1 Summary of basic concepts 4.1 基本概念の概略

(i) An alcohol-related disability is deemed to exist when there is an impairment in the physical, mental or social functioning of an individual, of such nature that it may be reasonably inferred that alcohol is part of the causal nexus determining that disability. Causal processes are usually complex and involve many interacting variables; attempts to quantify the degree of causality to be assigned to alcohol consumption are seldom useful. Assessment of degree of disability is best made within terms generally employed for that type of disability, without special regard to alcohol as antecedent. Any individual may experience a varied clustering of disabilities, and his total relevant disability experience should be described in the context of his general health and ill-health. Typologies and formal aggregation of alcohol-related disabilities are not of general usefulness.

(ii) The alcohol dependence syndrome is manifested by alterations at the behavioural, subjective, and psychobiological levels with, as a leading symptom, an impaired control over intake of the drug ethyl alcohol. The alcohol dependence syndrome exists in degrees. Its varied manifestations are influenced by modifying personal and environmental factors so as to give many different presentations. The alcohol dependence syndrome may usefully be viewed as one particular variety of alcohol-related disability: it is a disability which predicates the likelihood of drinking behaviour resulting over time in a clustering of other disabilities. The degree of disability in an individual that ensues from his alcohol dependence is determined by the general context of his health and ill-health: processes of interaction have to be gauged. The fact that an individual is alcohol dependent implies a probably impaired responsiveness of his behaviour to social control. Not all people manifesting alcohol-related disabilities are alcohol dependent, but they may be at increased risk of developing alcohol dependence. The role of gross alcohol consumption in increasing the risk should therefore also be considered, especially in the light of its implications for possible social as well as individual preventive measures.
