harmful use (F1x.1)有害な使用 (F1x.1)

A pattern of psychoactive substance use that is causing damage to health. The damage may be physical (e.g. hepatitis following injection of drugs) or mental (e.g. depressive episodes secondary to heavy alcohol intake). Harmful use commonly, but not invariably, has adverse social consequences; social consequences in themselves, however, are not sufficient to justify a diagnosis of harmful use.
The term was introduced in ICD-10 and supplanted “non-dependent use” as a diagnostic term. The closest equivalent in other diagnostic systems (e.g. DSM-IIIR) is substance abuse, which usually includes social consequences.

健康を害する原因となる精神作用物質の使用パターン。その害は身体的なもの(例:薬物注射によって引き起こされた肝炎)であるか、あるいは精神的なもの(例:アルコールの大量摂取の後の二次的な鬱エピソード)である。有害な使用は、必ずではないが一般的に、社会に対して悪影響を、またその人自身に社会的に重大な結果をもたらすが、この社会的な影響のみで有害な使用と診断することは正しくない。
この用語はICD-10にて導入され、それまでの診断用語としての「非依存使用(non-dependent use)」を置き換えた。他の診断システム(例:DSM-IIIR)における最も近い同義語は物質乱用であるが、そちらは社会的な悪影響を含めたものとなっている。
